In collaboration with the University of Colorado, etectRx has made significant strides in advancing medication adherence monitoring through the QUANTI-TAF study (NCT04065347). Recently published in Clinical Infectious Diseases1 and the British Journal of Clinical Pharmacology2, the QUANTI-TAF study focused on antiretroviral therapy (ART) adherence among persons with HIV (PWH) receiving daily oral tenofovir alafenamide/emtricitabine (TAF/FTC)-based ART for at least six months.

Over the course of the study, more than 9,000 ID-Cap® System ingestions contributed to a robust assessment of adherence. 84 participants actively used the ID-Cap System for approximately 4 months each. By combining the digital pill data with drug concentrations, the research team was able to accurately quantify tenofovir-diphosphate and emtricitabine-triphosphate in dried blood spots (DBS) at each visit according to adherence level1. Overall, adherence in the study was greater than 90%, and more than 90% of ingestions were detected by the ID-Cap System (as opposed to manually entered)2. The study also found that HIV-1 RNA (plasma viral load) remained suppressed (<200 copies/mL) even in cases of intermittent non-adherence, as measured by the ID-Cap System, reinforcing the potential for digital pill technologies in future research on HIV treatment strategies.

This study highlights the growing promise of digital health innovations by both enhancing medication adherence as well as offering critical insights into the effectiveness of ART regimens in real-world settings.

Our previous and ongoing HIV prevention clinical trials are just one example of the ID-Cap System making a difference in HIV PrEP non-adherence.

While the previous ID-Cap component design employed a wearable Reader in the form of a necklace, the evolution of the Wrist-Worn Reader provides a more functional and convenient solution. The Wrist-Worn Reader aims to revolutionize the ID-Cap System component capabilities by catering to different user preferences and lifestyles, making medication adherence technology more adaptable into everyday life. Preliminary testing results of the Wrist-Worn Reader showcase promising results for innovative technology to improve medication adherence and patient care. Adherence to medication regimens is critical for this population, but existing methods have limitations. The ID-Cap System addresses the challenge of consistent medication adherence and offers a precise solution by activating upon ingestion and transmitting adherence data to a cloud-based server, enhancing efficiency. The study also emphasizes the importance of understanding the target population’s perspectives and preferences to ensure the intervention’s real-world effectiveness. This research holds promise for improving medication adherence in a vulnerable patient group.

etectRx announces record-breaking use of the ID-Cap System, utilized in University of Colorado’s HIV treatment study. The Quantification of Tenofovir Alafenamide Adherence and Exposure in Adults living with HIV (QUANTI-TAF) study evaluated drug concentrations in dried blood spots (DBS), in tandem with medication adherence measured by the ID-Cap System.
A seamless, end-to-end solution, the ID-Cap is the only ingestible sensor solution which allowed the study’s researchers to monitor, track, trend, report, and provide real-time, accurate data through etectRx’s core technology, eBurst. When nonadherence was detected by the ingestible sensor technology, University of Colorado researchers were able to intervene in a timely fashion to send real-time dosing reminders to patients’ smartphone apps.

QUANTI-TAF began in August 2020, and the last follow-up concluded in May 2023. A total of 84 participants were enrolled in the study for up to 4 months each, resulting in 8,910 expected ingestions over 9,049 days monitored, and a cumulative adherence rate of 95%. The QUANTI-TAF study showed consistently high acceptability of the ID-Cap System over time, as participants used it for up to approximately 112 days (4 months).

Data from University of Colorado’s QUANTI-TAF study demonstrates significant milestones for the ID-Cap System with a reputation for demonstrating strong results and providing data to accurately track patients’ adherence to their oral medications. “The ID-Cap System was central to our QUANTI-TAF study; we used the digital pill data to establish adherence benchmarks for our drug concentrations, which was a key outcome of our study,” said Peter Anderson, PharmD- Antiviral Pharmacology, Professor at University of Colorado Anshutz Medical Campus.

etectRx gained FDA clearance for its innovative ID-Cap System technology in December 2019. Since FDA clearance, the technology has generated strong, accurate results across multiple indications, particularly with patient populations with traditionally poor levels of medication adherence, including those with substance abuse disorders.

The adherence intervention data provides encouraging results for this MSM population as substance use can increase PrEP nonadherence. RTM differs from existing Remote Patient Monitoring (RPM) in that it monitors therapeutic data, such as medication adherence. RPM measures physiologic data, such as blood pressure and heart rate.

Unlike the RPM codes, RTM services are available to Qualified Health Care Practitioners (“QHCPs”) who are not able to independently order and bill for E/M services. RTM is in an evolving state of development. In the near future, we can expect CMS will broaden its RTM coverage beyond musculoskeletal and respiratory disorders. In addition, as with most other CMS payment policy once there is significant RTM activity private insurers will adopt coverage for these services.
